Apakah Depresi Berpengaruh pada Derajat Nyeri Pasien Post-Pericutaneous Coronary Intervention?

Abstract


Salah satu metode terapi pada penyakit jantung yang banyak dilakukan adalah percutaneous coronary intervention (PCI). Masalah yang sering dihadapi pascatindakan PCI adalah post-procedure chest pain (PPCP), yaitu berbagai derajat nyeri dada yang tipikal maupun atipikal yang mulai muncul pascatindakan tersebut. Di sisi lain, ditemukan prevalensi depresi yang cukup tinggi di kalangan pasien yang menderita serangan jantung akut atau menjalani PCI. Penelitian ini bertujuan mengetahui pengaruh depresi terhadap derajat nyeri pada pasien yang menjalani prosedur PCI. Metode penelitian ini berupa analitik observasional rancangan potong lintang dengan melakukan analisis bivariat untuk melihat pengaruh depresi terhadap derajat nyeri dan analisis regresi logistik multinominal dengan tingkat signifikansi 5% untuk mengetahui variabel yang memengaruhi nyeri. Subjek pada penelitian ini adalah pasien yang menjalani prosedur PCI di unit Cathlab RSAU dr. M. Salamun pada 1 Juli 2023 sampai dengan 31 Juli 2023. Data dikumpulkan dengan kuesioner Beck’s Depression Inventory dan Numeric Rating Scale. Hasil penelitian didapatkan sebanyak 6 (9%) subjek yang mengalami depresi. Untuk pengukuran derajat nyeri didapatkan hasil 26 (38%) subjek mengalami derajat nyeri ringan, 41 (59%) nyeri sedang, dan 2 (3%) mengalami derajat berat. Pada penelitian didapatkan hasil likelihood ratio dengan nilai p=0,482 yang berarti tidak ada pengaruh depresi terhadap derajat nyeri dan nilai ρ-sig final sebesar 0,390 yang bermakna tidak ada variabel yang memengaruhi nyeri pada penelitian saat ini. Simpulan, tidak ada pengaruh depresi terhadap derajat nyeri pada pasien post-PCI. Hal ini mungkin disebabkan kaitan antara depresi dan nyeri sangat dipengaruhi oleh tipe nyeri sehingga harus dipahami dengan jelas mekanisme nyeri yang terjadi pada PCPP. Selain itu, perlu penelitian lebih lanjut untuk mengetahui faktor-faktor apa yang secara spesifik memengaruhi persepsi individu terhadap nyeri.

Abstract

One of the most common methods of treatment for heart disease is percutaneous coronary intervention (PCI). The problem that is often faced after PCI is post-procedure chest pain (PPCP), which is various degrees of typical and atypical chest pain that begins to appear after the procedure. On the other hand, there is a high prevalence of depression among patients who suffered an acute heart attack or underwent PCI. This study aims to determine the effect of depression on the degree of pain in patients undergoing PCI procedures. This research method is an observational analytic with a cross-sectional design, by conducting bivariate analysis to see the effect of depression on the degree of pain and multinominal logistic regression analysis with a significance level of 5% to determine the variables that affect pain. The subjects in this study were patients who underwent PCI procedures in the Cathlab unit of RSAU dr. M. Salamun on July 1, 2023 to July 31, 2023. Data were collected with Beck's Depression Inventory and Numeric Rating Scale questionnaires. The results showed that 6 (9%) subjects experienced depression. For the measurement of the degree of pain, it was found that 26 (38%) subjects experienced mild pain, 41 (59%) moderate pain and 2 (3%) experienced severe pain. The study obtained the results of the likelihood ratio with a value of p = 0.482 which means that there is no effect of depression on the degree of pain, and the final ρ-sig value of 0.390 which means that there are no variables that affect pain in the current study. The conclusion of this study is that there is no effect of depression on the degree of pain in post-PCI patients. This may be due to the relationship between depression and pain is strongly influenced by the type of pain so that it must be clearly understood the mechanism of pain that occurs in PCPP. In addition, further research is needed to find out what factors specifically affect individual perceptions of pain.
